En este artículo explico qué es, cómo se diagnostica y por qué, en los casos indicados, la prótesis trapeciometacarpiana es hoy la opción con la que consigo mejores resultados.If you feel pain at the base of your thumb when opening a jar, turning a key, or writing, you may have thumb base arthritis. It's one of the most common conditions I see in my clinic, and also one that responds particularly well to treatment when addressed at the right time. In this article I explain what it is, how it's diagnosed, and why, in the right cases, a trapeziometacarpal prosthesis is today the option that gives me the best results.

¿Qué es la rizartrosis?What is thumb base arthritis?

La rizartrosis es la artrosis de la articulación trapecio-metacarpiana, la que une la base del pulgar con el trapecio, uno de los huesos de la muñeca. Es una articulación en silla de montar que permite al pulgar moverse en muchas direcciones y hacer pinza con el resto de los dedos, lo que la somete a un desgaste considerable con los años. Es más frecuente en mujeres a partir de los 50 años, aunque también puede aparecer antes en personas con antecedentes familiares o tras un traumatismo previo en el pulgar.Thumb base arthritis is osteoarthritis of the trapeziometacarpal joint, which connects the base of the thumb to the trapezium, one of the bones of the wrist. It's a saddle-shaped joint that lets the thumb move in many directions and pinch against the other fingers, which puts it under considerable strain over the years. It's more common in women from around age 50 onwards, though it can also appear earlier in people with a family history or a previous thumb injury.

¿Cuándo se opera?When is surgery needed?

No toda rizartrosis necesita cirugía. En fases iniciales suelo recomendar una férula de descanso, ajustar las actividades que más cargan el pulgar y, en algunos casos, infiltraciones. La cirugía se plantea cuando el dolor limita el día a día del paciente pese a estas medidas, o cuando la artrosis está muy avanzada.Not every case of thumb base arthritis needs surgery. In early stages I usually recommend a rest splint, adjusting the activities that put the most strain on the thumb, and, in some cases, injections. Surgery is considered when pain limits the patient's daily life despite these measures, or when the arthritis is very advanced.

La prótesis trapeciometacarpianaThe trapeziometacarpal prosthesis

Durante años, la técnica de referencia para la rizartrosis avanzada ha sido la trapeciectomía: retirar el hueso trapecio desgastado. Es una técnica que funciona y sigue siendo una buena opción en muchos casos, pero suele requerir varias semanas de inmovilización y la recuperación de la fuerza es más lenta.For years, the standard technique for advanced thumb base arthritis has been trapeziectomy: removing the worn trapezium bone. It's a technique that works and remains a good option in many cases, but it usually requires several weeks of immobilisation, and strength recovers more slowly.

La prótesis trapeciometacarpiana sustituye la articulación desgastada por un implante, de forma parecida a como se hace en cadera o rodilla, pero a la escala del pulgar. Esto tiene una ventaja importante: al mantener la arquitectura de la articulación, la recuperación es mucho más rápida y, como comento también a mis pacientes con rizartrosis, no precisa inmovilización.The trapeziometacarpal prosthesis replaces the worn joint with an implant, in a similar way to a hip or knee replacement, but at the scale of the thumb. This has an important advantage: because it preserves the joint's architecture, recovery is much faster and, as I also tell my patients, it requires no immobilisation.

Esto no es solo impresión clínica: una revisión sistemática de 2025 que comparó ambas técnicas encontró que la prótesis ofrece mejores resultados funcionales a corto plazo y una recuperación más rápida, sobre todo en la fuerza, aunque conlleva el riesgo de complicaciones propias del implante que la trapeciectomía no tiene; esta, a cambio, ofrece resultados sólidos y duraderos a largo plazo (Datta et al., 2025, DOI: 10.7759/cureus.94908). Un estudio que comparó ambas cirugías en las mismas pacientes —una prótesis en un pulgar y una técnica de resección-suspensión en el otro— encontró más satisfacción y más fuerza de pinza y de agarre con la prótesis, y la mayoría de las pacientes, si tuvieran que elegir de nuevo, repetirían con el implante (Nietlispach et al., 2024, DOI: 10.1177/17531934241265809).This isn't just clinical impression: a 2025 systematic review comparing both techniques found that the prosthesis offers better short-term functional outcomes and a faster recovery, especially in strength, though it carries a risk of implant-specific complications that trapeziectomy doesn't have; trapeziectomy, in turn, offers solid, durable long-term results (Datta et al., 2025, DOI: 10.7759/cureus.94908). A study that compared both surgeries in the same patients — a prosthesis in one thumb and a resection-suspension technique in the other — found greater satisfaction and greater pinch and grip strength with the prosthesis, and most patients said that, given the choice again, they would choose the implant (Nietlispach et al., 2024, DOI: 10.1177/17531934241265809).

Dicho esto, conviene ser honesto con la evidencia: el registro británico de cirugía de la mano muestra que esa recuperación más rápida con la prótesis se nota sobre todo en los primeros 3 a 6 meses, y que a partir del año los resultados se igualan con la trapeciectomía (Hourston et al., 2026, DOI: 10.1142/S2424835526500062). Y la revisión Cochrane más reciente sobre la cirugía de la rizartrosis, la referencia de mayor calidad metodológica disponible, señala que la certeza de la evidencia al comparar la prótesis con la trapeciectomía con reconstrucción ligamentosa todavía es muy baja (Baljer et al., 2026, DOI: 10.1002/14651858.CD004631.pub6). Esto no invalida lo que veo en consulta, pero sí explica por qué no todos los cirujanos de mano recomiendan la prótesis por igual, y por qué decidir la técnica caso a caso —según el grado de artrosis, la calidad del hueso y la actividad de cada persona— sigue siendo lo más sensato.That said, it's worth being honest about the evidence: the UK hand surgery registry shows that this faster recovery with the prosthesis is mostly noticeable in the first 3 to 6 months, and that by one year the results even out with trapeziectomy (Hourston et al., 2026, DOI: 10.1142/S2424835526500062). And the most recent Cochrane review on thumb base arthritis surgery, the highest-quality evidence available, notes that the certainty of the evidence comparing the prosthesis with trapeziectomy plus ligament reconstruction is still very low (Baljer et al., 2026, DOI: 10.1002/14651858.CD004631.pub6). This doesn't invalidate what I see in clinic, but it does explain why not every hand surgeon recommends the prosthesis equally, and why deciding on a case-by-case basis — depending on the degree of arthritis, bone quality, and each person's activity level — remains the most sensible approach.

Recuperación y resultados esperablesRecovery and expected results

Al no necesitar inmovilización, recomiendo mantener la mano elevada los primeros días para controlar la inflamación, e iniciar cuanto antes la movilización progresiva del pulgar, siempre acompañada de rehabilitación según el grado de afectación de cada paciente. La mayoría recupera el uso funcional de la mano en pocas semanas, y la fuerza de pinza sigue mejorando durante los meses siguientes.Since no immobilisation is needed, I recommend keeping the hand elevated for the first few days to control swelling, and starting progressive movement of the thumb as soon as possible, always alongside rehabilitation suited to each patient. Most patients regain functional use of the hand within a few weeks, and pinch strength keeps improving over the following months.
